1. Assumption of Risk`

I, the undersigned parent or legal guardian of the above-named child, understand that participation in classes, activities, physical activities, and related events (collectively, the “Activities”) involves certain inherent risks, including but not limited to minor injuries, accidents, illness, property damage, or other unforeseen risks.

I voluntarily assume full responsibility for any risks of loss, property damage, injury, or illness that may be sustained by my child as a result of participation in the Activities.

2. Release and Waiver of Liability

I hereby release, waive, discharge, and hold harmless Homeschool Enrichment Community (HEC), its owners, instructors, , assistants, facility providers, and representatives from any and all claims, demands, actions, causes of action, damages, or liabilities arising out of or related to my child’s participation in the Activities, including claims resulting from ordinary negligence.

3. Behavior Expectations

I understand that my child is expected to behave respectfully and follow all safety rules. The homeschool program reserves the right to dismiss any student whose behavior is unsafe or disruptive.

I understand that this is not a drop-off off program and a parent or guardian must remain on the property when their children are present.

I agree to not bring my child to a class with any severe and/or contagious illness.

4. Photo/Media Release

5. Acknowledgment

I have carefully read this Liability Waiver and Release and fully understand its contents. I understand that I am giving up certain legal rights by signing this document, and I sign it voluntarily.
